To apply for MCCCC, Inc. services:
Applicant must live in Mid Chambers County, complete the application form and qualify
Items needed from you for the voucher program are:
Current driver’s license or Department of Public Safety ID card on all adults in the
household. (Must have Mid Chambers address) Most recent Electric Bill -- name and
address must matching the name and address on Photo ID. Homes sharing utilities
will be considered one household.
Provide proof of income. Last four pay check stubs or statement from employer;
award letter showing amount of SSI/SS, pension benefits, veteran benefits, workers compensation or unemployment; current year’s Income Tax.
Proof a child is related to you – Legal Birth Certificate.
If claiming children other than your own, must provide information on parents or have proof of guardianship.
Applicant must complete the income and expense section of application. If someone else pays your bills, we must have Photo ID of that person with current address.
Providing false information or incomplete information will cause the recipient and their household (physical address) to be disqualified from the Mid Chambers Christian Caring Center Inc. programs.
Qualifying seniors may receive 8 - $25 vouchers per year — no more than one per month.
Qualifying persons (ages 20 – 64) may receive up to 36 - $25 vouchers until their 65th birthday – no more than 6 vouchers per year; no more than one per month. Trinity Bay and City of Anahuac water vouchers – bill must have the same name and address as the Photo ID.
J J Chevron gasoline voucher - recipient must have a letter on the doctor’s letterhead stating time and date of doctor’s appointment. Doctor’s appointment must be for a person listed on the MCCCC Inc. application form. Doctor’s appointment must be out of town.
Wilcox Drug Store voucher – medical supplies/prescription must be filled at Wilcox. Prescription must be for a person listed on the MCCCC Inc. application form.
Clients must provide proof of residence and complete the USDA Texas Commodity Assistance Program (TEXCAP) Form H1555 and meet the income guideline on Form H1640.
